Medical Home Recognition
Consultation

We will assist and guide your practice through the recognition and accreditation process utilizing our practice transformation initiatives. You will be provided with the following:

The tools to accomplish the National Committee of Quality Assurance (NCQA) Patient-Centered Medical Home recognition.

One-on-one support to transform your practice to improve patient quality of care, and take advantage of financial incentives that are available.

Proven ability to guide both multi-location and single site practices through the recognition process.

Strategic Planning

We offer Electronic Health Record (EHR) Implementation Strategies for practices who are interested in preparing themselves to meet the requirements as issued under the HITECH Act and optimize EHR usage.

Meaningful Use

We will consult with your office to evaluate your current operations and develop a plan to optimize your EHR usage in order to meet the Meaningful Use criteria. Early adopters are eligible for higher reimbursements. Benefits are available in 2011 and are currently set to expire in 2014. In subsequent years a penalty will be assessed if not met.

Physician Quality Reporting Initiative (PQRI)

Our trained staff will work with you to select appropriate clinical reporting measures for your practice and implement them in the clinical setting. Following implementation we will ensure successful transmission to Medicare in order to receive financial incentives.

ePrescribing

We will work with your providers to implement electronic prescribing in a two-way communication between prescribers, pharmacies and pharmacy benefit managers. This will enable you to take advantage of government reimbursements before it’s too late.
